“Keeping Wakeman a Safe Community”
To enter the Vial of Life Program, all you have to do is fill out the piece of paper that we supply with your medical information. Place the paper in a pill vial that we also supply and place the vial in the refrigerator door inside your home.
We will know right where to look.
This vial could help emergency personnel evaluate your condition more effectively and help us to locate your family immediately if we need to.
This program is available to all Wakeman Residents at no cost. This will be especially effective for residents who live alone. The vials are available at the Wakeman Police Department, Citizens Ambulance,
and the Wakeman Area Family Medical Center.

Forms to download. The forms are available in four formats. The PDF is the most universal, but you will need the Adobe Acrobat Reader to open it. You can download it free by clicking here. The second should open in your word processing program. The third option will open in Corel Word Perfect. The fourth option is a form that you can fill in by typing in the form field and then printing the page.
